Initial Consultation

What happens during my consultation?

Your initial office visit will usually consist of a consultation during which the doctor will evaluate your needs and explain your diagnosis. The doctor will also discuss your treatment options.

Occasionally minor procedures can be done on the same day as your initial evaluation and consultation. However, a complex medical history or surgical treatment plan will require an initial evaluation with a second appointment scheduled to provide the treatment on another day.

What do I need for my initial consultation?

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip
  • Any current radiographs (x-rays) if applicable
  • A list of medications you are currently taking
  • If you have applicable medical or dental insurance, bring the necessary completed forms with you. This will save time an allow us to help you with your claims.


IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian at the examination and consultation visit.

A pre-operative consultation and evaluation is mandatory for patients undergoing IV anesthesia for surgery. This evaluation must be done on a day prior to your surgery and anesthesia. Special preparation is necessary when IV anesthesia or sedation is plan.

Radiographs (x-rays)

If your dentist or physician has taken any x-rays, you may request that these are forwarded to our office. If there is not enough time for delivery by mail, please contact your dentist/physician so that you may pick up the x-rays and bring them to our office. If additional films or studies are necessary, they can be taken at our facility.
